1. ONLINE FORMS:
   • If online form is available, fill it out and
   • Click “SUBMIT”
2. PAPER FORMS:
   • Print the form
   • Fill out the form; make sure it is notarized/signed as required
   • When form is complete:
     • Scan and email to [email protected]; or
     • Drop off at the Church Office
